CONSENT AGREEMENT

FOR ELECTRONIC DELIVERY OF STATEMENTS AND OTHER RECORDS

 

I authorize First Nations Bank to deliver correspondence and other communications including account statements, notices, alerts, check images, and other official documents required in connection with my First Nations Bank account.  This may include, but is not limited to, all legal and regulatory disclosures and communications associated with my account, notices or disclosures about a change in the terms of my account, and privacy policies.  The terms and conditions of the deposit agreements and disclosures for each First Nations Bank account, as well as my other agreements with First Nations Bank, will continue to apply.

 

DELIVERY OF ONLINE STATEMENTS

In order to receive your statements online, you must be enrolled in First Nations Bank Online Banking.  Once enrolled in Online Banking and Online Statements, you will no longer receive them in paper form. 

 

You will receive an email when your statement is available for viewing.  To view the online statement, log into First Nations Bank’s Online Banking.  The available online statements will be displayed under the Reports link in the Top Navigation bar. The online statements for your account(s) will be available on-line for 18 months.  The 18 months will start from the date of enrollment.  The statements will be in a portable document file (PDF).  Adobe Acrobat Reader is required for Online Statements and can be downloaded free of charge at http://adobe.com/products/acrobat. 

 

To retain a copy of the statements, disclosures, agreements and required notices, you will need a media storage device (e.g., hard drive) and/or a printer to print copies of the documents. You also have a right to obtain paper copies of your current and prior account statement(s) and required notices by contacting Customer Service by phone at 773-594-5900. We will charge the Bank Statement Reproduction fees as set forth in the Bank’s current Common Features disclosure which may be amended by the Bank at any time. 

 

Online Banking is generally available 24 hours a day, seven days a week; however, service may be unavailable from time to time for routine software and hardware maintenance or due to unscheduled down time.

 

TERMINATION OR DISCONTINUTATION

If you wish to discontinue the Online Statement service, you must change your selection through Online Banking by clicking on the Reports Link and selecting Statements and Documents.  Click the view and maintain documents preferences link.  By changing an account's statement delivery method from Online to Paper, you will lose access to any statements previously delivered online. You may want to save your online statements before making this change.  First Nations Bank may suspend or terminate your Online Statement service at any time without notice.

 

You may be charged a paper statement fee if you choose to terminate your enrollment in Online Statements.  Please see your deposit account agreement and common features disclosure for applicable fees.  Termination of your enrollment in the Online Statements service will not affect your enrollment in the First Nations Bank’s Online Banking service.

 

CHANGE OF EMAIL

You agree to immediately notify First Nations Bank of any change in the email address registered to the Online Banking under the Administration, Self Administration, Personal Preferences, Email section.

 

CHANGE IN TERMS

You understand that the Bank may change this Agreement at any time.  Changes to this Agreement will be effective immediately after the Bank makes them, unless applicable law requires the Bank to provide me with advance notification.  Those changes will be effective immediately after advance notice has been issued or upon the date stated in such notice, as required by applicable law.  If the changes would result in any increased fees or liability to me, the Bank agrees to provide notice at least thirty (30) days prior to the effective date. 

 

COMPUTER HARDWARE AND SOFTWARE REQUIREMENTS

The Digital One platform is compatible with the following browsers and operating systems:

Supported browsers and operating systems can operate on desktop, tablet, and smartphones. Content is sized and displayed based on the screen resolution and other display attributes of the device.

 

Note: Safari cannot be used with a signature pad or peripheral devices for Digital One Teller.

 

Microsoft Internet Explorer® 11 (IE11) for the Windows 10 operating system is only supported for Digital One Banker. IE11 is no longer applicable to any other Digital One channel.

 

 

 

SECURITY MEASURES

The Bank will use reasonable measures, consistent with industry standards to maintain an appropriate level of security over the information contained in your Online Statement.  The Bank has no control over the persons who have access to your personal computer and Online Statements once they are in your possession.  The Bank will not be liable for any unauthorized access to your personal computer or to the information maintained on your computer, including Online Statements.  You are responsible to initiate and maintain adequate procedures to prevent unauthorized access to your personal computer or unauthorized use of your electronic or other files, including your Online Statement.  The industry standards for electronic technology are constantly changing and new standards are being developed.  You acknowledge by consenting to this Authorization that there are risks to the electronic delivery of your Online Statements, including but not limited to: 1) delay or failure due to technical difficulties, 2) weather conditions, 3) matters beyond the Bank’s reasonable control, 4) alteration of bank statements by third parties in spite of the Bank’s reasonable security measures.  By consenting to this Agreement, you represent to the Bank that you have considered the Bank’s security measures and find these security measures to be reasonable.  You should consider the content of your Online Statements, the risks associated with the electronic delivery of your Online Statements, and the Bank’s security procedures.

 

CONFIRMATION OF CONSENT

I consent that I have read the above and authorize First Nations Bank to electronically deliver statements and other records regarding my accounts with First Nations Bank and I agree to the terms and conditions of this Agreement.  My electronic signature shall be legally binding and any transaction performed pursuant to this Consent Agreement shall be considered authorized by me.
